The straight answer
El Fuerte is a quiet pueblo, and the historic center feels calm. Walking the plaza, the church, the fort-museum and the surrounding streets by day is relaxed and normal. Evenings on the square are fine too; families and other travelers are out, and the core stays lit and low-key.
Day and night
Stick to the center after dark. The lit blocks around the plaza and your hotel are the parts you want, and they are easy to cover on foot. Once you drift into the unlit residential streets on the edges of town at night, there is no reason to be there and little to see, so head back toward the square. Nothing dramatic, just common sense in a small town where streets go dark early.
The real risks
The honest risks here are ordinary. Uneven cobblestones and dim streets mean watch your footing at night. On the river tour, listen to your boatman and mind the water rather than the wildlife photo. Petty theft is low but not zero, so do not leave bags unattended on the plaza.
What a local would tell you
This is northern Sinaloa, and regional conditions on the roads between towns can shift. The town itself is calm; the thing to check is overland travel. Ask your hotel about current conditions before driving highways after dark, and prefer daytime for any road trip in or out.
